The Patient Portal
Our goal at Soni Family Medicine is to provide our patients with the best care possible. Using a Patient Portal is an important part of accomplishing that goal. You will be able to easily set up appointments, directly access your health records, communicate securely with Dr. Soni, and more!
The Patient Portal that we use is called Patient Fusion. (It is provided by our electronic medical records company, www.PracticeFusion.com.) Dr. Soni has worked with three different EMRs over the past 10 years, and found that the user interface of Patient Fusion is what works best for his patients.
In order to set up your account on Patient Fusion, please follow the steps below. At any time, if you have any questions or want help, please contact us via email at help@SoniFamillyMedicine.com or by phone at (480) 385-9896.
If you prefer to get help in person, we are happy to guide you through this sign-up process in the office. Just call us at (480) 385-9896 to set up an appointment.

How to Sign Up
+ Step 1: Request an Invitation to Join the Patient Portal
Send an email to us at help@SoniFamilyMedicine.com. Please copy and paste the following information into your email:
SUBJECT: Request to join Patient Portal
MESSAGE: I would like to join the Patient Portal. Here is the information that you require in order to send me an invitation:
Full Name: First Last
Date of Birth: mm/dd/yyyy
Cell Phone: (xxx) xxx-xxxx (faster method - receive security code via text message)
Home Phone: (xxx) xxx-xxxx (if you do not have a cell phone)
+ Step 2: Receive an Email from Patient Fusion
Within 24 hours, Soni Family Medicine will request that Patient Fusion send an invitation to you. Be on the lookout for an email from Patient Fusion with the subject line: "[Your First Name], your health records are now available from Soni Family Medicine." Open the message and click on the orange button that says "Access your records."
+ Step 3: Select a Method to Verify Your Identity
A pop up window labeled "Verify identity" will open. The following options will be listed:
o Enter PIN received from the doctor
o Request security code via phone
Select the method that you prefer. (It will be faster if you "Request security code via phone," but you must have a cell phone capable of receiving text messages in order to use this method.) Then click on the orange button labeled "Continue."
+ Step 4: Verify Your Identity
If you opted to "Enter a PIN received from the doctor," a pop up window labeled "Verify identity using PIN" will open. You will be asked to enter: "Patient's date of birth" and "PIN received from patient's doctor." You do not yet have the PIN - a staff member from Soni Family Medicine must call you to give you the PIN. (We apologize for this delay, but for security reasons, we are not allowed to send the PIN to you through email. You will receive a call from us within 24 hours.)
If you chose to "Request security code via phone," a pop up window labeled "Verify identity using security code" will open. You will be asked to enter: "Patient's date of birth" and "Your phone number from the doctor's record" (Be sure to enter your cell phone number.) Then click on the orange button labeled "Request code." Shortly thereafter you will receive a text message with the security code you need to enter.
+ Step 5: Set Up an Account
Next, a pop up window labeled "Set up account" will open with the text "Your identity has been verified" inside a box outlined in green. The following options will be listed:
o Link to an existing Patient Fusion account
o Create a new account
Select "Create a new account" and click on the orange button labeled "Continue."
+ Step 6: Opt In/Out of Text Messages
A pop up window labeled "Welcome to your Patient Fusion account" will open. Select whether you would like to receive text messages or not, and then click on the blue button labeled "Continue."
+ Done
Your Patient Portal page will open up. Using the navigation bar on the left-hand-side, you can check your:
Health Records (including lab results, medications, vitals, etc.)
Appointments
Providers
Profile
Account Settings
To set up your next appointment, select "Appointments" in the navigation bar, and then click on the blue "Book appointment" text in the upper right-hand-corner. We look forward to seeing you soon!
